The Centers for Medicare and Medicaid Services (CMS) requires hospitals to make available online a listing of standard hospital charges using a CMS adopted layout with charge information encoded in accordance with CMS technical specifications.  CMS also requires the publishing of 300-shoppable services.  Crawford Memorial Hospital supports the intent of these regulations to educate patients on the cost of healthcare and the patient financial responsibilities.  Crawford Memorial Hospital has made good faith effort to ensure the data in the price transparency files is true, accurate and complete. Please use the links below to view our chargemaster and contracted rates.

FINANCIAL INFORMATION
If your care at Crawford Memorial Hospital is planned in advance, you should take the opportunity to look over your health care insurance, Medicare, Medicaid or other assistance coverage. Please call your insurance company to make sure that your services will be covered and to get approval for any procedures that require pre-certification.

Understanding what portion of the costs will be your responsibility can help you and your family plan effectively for the cost of your care. Most insurance plans do not cover 100% of your hospital stay or other visits. Co-payments and deductibles may be due at time of service. If you are unable to pay a large balance due, you may make arrangements for installment payments by calling the financial counselors at 618-546-2568.

If you have no insurance, you may be asked to pay a deposit for elective services upon admission. For those uninsured patients that are ineligible for financial assistance, CMH offers an uninsured discount and a prompt payment discount.

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.
